Application to Note Change/Correction of Name (A4)

When a registered proprietor's name changes or was recorded wrongly, the Register Book must be updated before most other dealings can be registered in the new name. The Registrar-General can correct errors and update information recorded in the Register Book and on certificates (Real Property Act 1886 (SA) s 220(f)). Form A4 is used for a change of name by a person (formal change, marriage, reverting to a former surname, alteration or correction), a company or an association. Use prefix CN for a freehold or Crown lease title and CN2 for a subsidiary interest such as a lease, mortgage or encumbrance. Like other registry instruments it needs the certifications in section 273, and Land Services SA's verification of identity and authority requirements apply.

When you need it

• You changed your name by marriage, deed poll or registration with Births, Deaths and Marriages, or reverted to a surname used before marriage.
• Your name was misspelt or recorded incorrectly on the title, for example through an error in a transfer.
• A company or incorporated association that holds land or a registered interest has changed its name.

   Land description

   For a title, write 'Whole of the land in CT Volume ... Folio ...'. For a subsidiary interest, identify it, for example 'Mortgage (number) registered on CT Volume ... Folio ...'.

   Registered name and applicant

   Insert the full name exactly as it appears on the title or instrument, then the applicant's current full name and address. Each applicant needs a separate form.

   Reason for the change

   Explain why, for example 'as a consequence of marriage', 'change of name by deed poll', 'error in transfer 12334455' or a name change under the Corporations Act 2001 or the Associations Incorporation Act 1985.

   Supporting evidence and certification

   List the evidence lodged, such as a copy of a marriage or change of name certificate. Complete the certification. A self-represented applicant makes only the certifications about retaining evidence and the document being correct and compliant; a lawyer or conveyancer makes them all.

• Change of name dealings executed on or after 8 April 2024 must be lodged electronically by industry professionals through an Electronic Lodgement Network Operator, unless an exemption applies (for example correcting the split or order of given names and surname, or a company correcting its ACN). A paper channel remains for self-represented parties.
• Self-represented applicants lodge in person at Level 9, 101 Grenfell Street, Adelaide, or by post to GPO Box 543, with the evidence of the change and evidence of identity and authority.
• A registration fee applies; check Land Services SA's fees page. It can be paid by EFTPOS or cash in person, or by cheque; if not paid at lodgement, you are emailed a secure online payment link.

• Self-represented applicants must meet the verification of identity requirements (including certified copies of identity documents) and have an Authorised Person Certification completed by someone such as a justice of the peace.
• Verification of authority needs two documents linking you to the property, such as current council rates, ESL notice, land tax or utilities bills; photocopies are accepted.
• Land Services SA will not accept the dealing if the identity and authority requirements are not met.
