Form B15 is the Federal Court of Australia petition by an executor or administrator to have a deceased estate administered in bankruptcy. It is made under s 247 of the Bankruptcy Act.

Form 59 is the approved affidavit form for the Federal Court of Australia under rule 29.02 of the Federal Court Rules 2011. It sets out a witness's evidence in numbered paragraphs, sworn or affirmed.

Form 40 sets out a party's written answers to interrogatories in the Federal Court of Australia, with the affidavit verifying them, under rule 21.03.

Form B14 is a creditor's petition asking the Federal Court of Australia to order that a deceased person's estate be administered under the Bankruptcy Act.

Form 67 is the Federal Court of Australia application for more time to file a notice of appeal or cross-appeal, or to start judicial review or an ART appeal.

Form 22 is used in a Federal Court of Australia class action to seek procedural orders under ss 33K, 33W, 33X or 33ZA of the Federal Court of Australia Act.

Form 118 is the Federal Court of Australia application for extension of time and leave to appeal under rule 35.14(1) of the Federal Court Rules 2011. It is used once the 14 day limit has passed.

Form 117 is the Federal Court of Australia application for leave to appeal, used where an Act requires leave, such as an appeal from an interlocutory judgment.

Form 133 asks a Federal Court of Australia Judge to review a taxing officer's taxation of costs. It must be filed within 28 days after the certificate of taxation issues.

Form B10 is the Federal Court of Australia application for a s 81 Bankruptcy Act summons to examine a bankrupt or an examinable person about the bankruptcy.

Form 115 asks the Federal Court of Australia for leave under s 85 of the Native Title Act 1993 for a party to be represented by someone who is not a lawyer.

Form B17 is the Federal Court of Australia warrant under s 264B of the Bankruptcy Act to apprehend a person who fails to attend under a bankruptcy summons.

Form B16 is the Federal Court of Australia arrest warrant under s 78 of the Bankruptcy Act. A Judge issues it for a debtor or bankrupt who absconds or hides property.

Form 99 is the Federal Court of Australia certificate that a person served in New Zealand with a subpoena, by leave, failed to comply with it.

Form 132 is the Federal Court of Australia certificate of taxation, issued by a taxing officer to fix the amount of costs payable. It has the effect of a Court order.

Form B19 is a registered trustee's consent to be designated by the Federal Court of Australia to deal with a debtor's assets. It is used under the Model Law on Cross-Border Insolvency.

Form B6 is the Creditor's petition a creditor files in the Federal Court of Australia to ask for a sequestration order making a debtor bankrupt.

Form 33 is the Federal Court of Australia Defence under rule 16.32 of the Federal Court Rules 2011. A respondent files it within 28 days after being served with a statement of claim to answer each allegation.

Form 103 is the Federal Court of Australia petition to dispute a Torres Strait Regional Authority election. It is made under the Aboriginal and Torres Strait Islander Act 2005.

Form B3 is the Interim application used in a Federal Court of Australia bankruptcy proceeding that has already started. It sets out the interim orders sought.

Form 35 is the Interlocutory application used to ask the Federal Court of Australia for an order in a proceeding that has already started, under rule 17.01.

Form 97 is the Federal Court of Australia interlocutory application for leave to serve a subpoena in New Zealand under the Trans-Tasman Proceedings Act 2010.

Form 102 asks the Federal Court of Australia for an order that evidence be taken, or submissions made, by audio or audiovisual link from New Zealand.

Form 111 asks the Federal Court of Australia, in a native title case, for orders that respect cultural or customary concerns, such as naming deceased people.

Form 105 is the Federal Court of Australia application to join a native title claim as a party after the 3-month notification period has ended.

Form 38 is the list of documents used to give discovery in the Federal Court of Australia. It is verified by an affidavit and served on the other parties, not filed.

Form 106 is the Federal Court of Australia notice a non-applicant party gives to stop being a party to a native title proceeding. It must be given before the first hearing.

Form 18 is the Federal Court of Australia notice that a proceeding involves a constitutional matter under section 78B of the Judiciary Act 1903. It alerts the Attorneys-General.

Form 46 is the Federal Court of Australia notice used to accept a formal offer to compromise. The offeree serves it on the party who made the offer while the offer is open.

Form 4 is the Federal Court of Australia notice a lawyer files when a party who started the proceeding unrepresented later appoints a lawyer. It also gives the new address for service.

Form 5 is the Federal Court of Australia Notice of acting - change of lawyer under rule 4.04(1) of the Federal Court Rules 2011. The new lawyer files it when a party ends one lawyer's retainer and appoints another.

Form 10 is the Federal Court of Australia Notice of address for service under rules 5.02 and 11.07 of the Federal Court Rules 2011. A respondent files it to say where documents can be served.

Form 37 is the Federal Court of Australia notice an execution creditor serves on the Sheriff to admit a third party's claim to property taken in execution.

Form 122 is the Federal Court of Australia notice of appeal from a single Federal Court judge or from another court. It is filed within 28 days of the judgment.

Form 92 is the Federal Court of Australia notice of appeal from a decision of the Commissioner of Patents or the Registrar of Trade Marks or Designs. It is filed within 21 days.

Form 73 is the Federal Court of Australia notice of appeal against a tax objection decision under section 14ZZ of the Taxation Administration Act 1953. It is filed within 60 days.

Form 74 is the Federal Court of Australia notice of appeal against a departure prohibition order made by the Commissioner of Taxation under section 14V.

Form 75 is the Federal Court of Australia notice of appeal on a question of law from the Administrative Review Tribunal. It is also used for AFCA and NNTT appeals.

Form 121 is the Federal Court of Australia notice of appeal from a Federal Circuit and Family Court (Division 2) judgment. It is for general federal law, not family law.

Form B4 is the notice of appearance under the Federal Court (Bankruptcy) Rules 2016. It is filed by anyone who intends to appear at a bankruptcy hearing or examination.

Form 113 is the Federal Court of Australia notice that a native title party has appointed an organisation as its agent under section 84B. It is filed within 14 days.

Form 8 is the Federal Court of Australia notice a lawyer files to record that they no longer act for a party, giving the party's last known address.

Form 114 is the Federal Court of Australia notice of a change to a native title party's section 84B agent, or the appointment of a new agent. It is filed within 14 days.

Form 28 is the Federal Court of Australia notice a party files to change its address for service in a proceeding. A copy is then served on every other party.

Form 36 is the Federal Court of Australia notice a person gives the Sheriff to claim property taken, or about to be taken, in execution against someone else.

Form 20 is the Federal Court of Australia notice by which a government, Minister or public body consents to be a group member in a class action.

Form 124 is the Federal Court of Australia Notice of contention under rule 36.24 of the Federal Court Rules 2011. An appeal respondent uses it to argue the judgment should stand on other grounds.

Form 77 is the Federal Court of Australia notice of contention in tribunal and IP Commissioner appeals. It says the decision should stand on other grounds.

Form 123 is the Federal Court of Australia notice of cross-appeal in an appeal from a court. A respondent uses it to have part of the judgment or an order changed.

Form 93 is the Federal Court of Australia notice of cross-appeal from a decision of the Commissioner of Patents or the Registrar of Trade Marks or Designs.

Form 76 is the Federal Court of Australia notice of cross-appeal for a respondent to an appeal from the Administrative Review Tribunal or a similar tribunal.

Form 31 is the Federal Court of Australia notice of cross-claim, used by a respondent to claim relief against the applicant, another respondent or a new party.

Form 48 is the Federal Court of Australia notice of discontinuance, used by a party claiming relief to end a proceeding or cross-claim, in whole or in part.

Form 126 is the Federal Court of Australia notice used to discontinue an appeal or cross-appeal from a court. It has the effect of an order dismissing the appeal.

Form 78 is the Federal Court of Australia notice to discontinue an appeal or cross-appeal from a tribunal. It covers appeals from the Administrative Review Tribunal, AFCA and the National Native Title Tribunal.

Form B22 is the Federal Court of Australia notice to creditors that an application to recognise a foreign bankruptcy proceeding has been dismissed or withdrawn.

Form 42 is the Federal Court of Australia notice of dispute, served in reply to a notice to admit to dispute facts or the authenticity of documents. Serve it within 14 days.

Form 72 is the notice a Federal Court Registrar attaches when the High Court refers an election petition to the Federal Court of Australia. It sets the hearing and service steps.

Form B20 is the Federal Court of Australia notice to creditors of an application to recognise a foreign insolvency proceeding. It is used where the debtor is an individual.

Form B23 is the Federal Court of Australia notice to creditors of an application to modify or end a cross-border insolvency order. It covers recognition orders and Model Law relief.

Form 65 is the Federal Court of Australia coincidence notice under section 98(1) of the Evidence Act 1995. Give it before relying on the improbability of similar events occurring by chance.

Form 62 is the Federal Court of Australia hearsay notice under section 67 of the Evidence Act 1995. Give it before relying on the civil hearsay exceptions in section 63(2) or 64(2).

Form 64 is the Federal Court of Australia tendency notice under section 97(1) of the Evidence Act 1995. Give it before leading evidence that a person tends to act a certain way.

Form 7 is the Federal Court of Australia notice a lawyer serves on a client before ceasing to act. It warns the client to file their own address for service once the lawyer stops acting.

Form B21 is the Federal Court of Australia notice to creditors of an order made under the Cross-Border Insolvency Act 2008. Examples are recognition orders and Model Law relief.

Form 119 is the Federal Court of Australia notice objecting to an application, such as for leave to appeal, being decided on the papers without an oral hearing.

Form 130 is the Federal Court of Australia notice of objection to items in a bill of costs that is going to taxation. File and serve it at least 14 days before the taxation date.

Form 68 is the Federal Court of Australia notice of objection to competency for judicial review applications, migration applications and tribunal appeals. File it within 14 days of service.

Form 125 is the Federal Court of Australia notice of objection to competency of an appeal from a court. A respondent files it within 14 days after the notice of appeal is served.

Form 128 is the Federal Court of Australia notice objecting to a Registrar's estimate of a bill of costs. File it within 21 days of the notice of estimate and pay $2,000 security.

Form 63 is the Federal Court of Australia notice objecting to hearsay evidence under section 68 of the Evidence Act 1995. Use it to have an available witness called instead.

Form 45 is the Notice of offer to compromise under rule 25.01 of the Federal Court Rules 2011 (Cth). A party serves it to make a formal settlement offer that can carry costs consequences if not accepted.

Form 71 is the notice a Federal Court Registrar attaches to a High Court order remitting a proceeding to the Federal Court of Australia. It fixes the return date and place of hearing.

Form 49 is the notice a Federal Court Registrar attaches to an order transferring a proceeding into the Federal Court of Australia. It sets the first hearing and asks parties for an address for service.

Form 131 is the Federal Court of Australia notice of response to an objection to a bill of costs. It says which objections are admitted and why the rest should fail.

Form 6 is the Federal Court of Australia notice a party files after ending their lawyer's retainer without appointing a new lawyer. It also gives the party's own address for service.

Form 120 is the Federal Court of Australia notice withdrawing an application for leave to appeal, or for more time to seek leave, in whole or in part.

Form 47 is the Federal Court of Australia notice of withdrawal of pleading under rule 26.11. Use it to withdraw all or part of a plea, such as paragraphs of a defence.

Form 129 is the Federal Court of Australia notice requesting a full taxation of costs after a bill has been provisionally taxed. File it within 21 days of the provisional taxation notice.

Form 32 is the Federal Court of Australia notice a cross-respondent files to require service of pleadings and documents filed before the cross-claim.

Form B5 is the Federal Court of Australia bankruptcy notice of grounds of opposition to an application or creditor's petition. File it with an appearance and affidavit.

Form 41 is the Notice to admit used in the Federal Court of Australia under rule 22.01. A party serves it to ask another party to admit facts or the authenticity of documents.

Form B13 is the Federal Court notice to creditors of a hearing about a debt agreement or personal insolvency agreement. It is required by rules 9.04 and 10.04 of the Bankruptcy Rules.

Form B11 is the Federal Court notice to creditors of an annulment application under the Federal Court (Bankruptcy) Rules 2016. It tells creditors a bankruptcy annulment hearing is coming.

Form B12 is the Federal Court notice to creditors that a Registrar's decision to make a sequestration order is being reviewed. It is required by rule 7.05 of the Bankruptcy Rules.

Form 61 is the Federal Court of Australia notice to produce under rule 30.28. A party serves it on another party to require documents or things to be produced at a trial or hearing.

Form 39 is the Federal Court of Australia notice to produce a document mentioned in a pleading or affidavit (r 20.31). It asks the party who filed it to make the document available for inspection.

Form 135 is the Federal Court of Australia order registering a foreign money judgment under rule 41.64(1) of the Federal Court Rules 2011. It applies the Foreign Judgments Act 1991 (Cth).

Form 136 is the Federal Court of Australia order for registration of a non-money foreign judgment under the Foreign Judgments Act 1991. Rule 41.64(2) sets the form the order takes.

Form 15 is the general originating application for starting a proceeding in the Federal Court of Australia (rule 8.01, Federal Court Rules 2011). It sets out the relief you claim.

Form 104 is the Federal Court of Australia reference of a question about Torres Strait Regional Authority membership. The Minister uses it to ask whether a member is qualified or a seat is vacant.

Form 86 is the Federal Court of Australia application for interim orders while an inquiry into a registered organisation election is under way. It is made under s 204 of the RO Act.

Form 60 is the Federal Court of Australia application to take and preserve evidence of a future right or claim. It is made under rule 29.23 of the Federal Court Rules 2011.

Form 87 is the Federal Court of Australia application to halt an uncompleted amalgamation ballot during an inquiry. It is made under reg 77(1) of the RO Regulations.

Form 94 is the Federal Court of Australia application for a determination of equitable remuneration under s 20(2) of the Circuit Layouts Act 1989 (rule 34.32).

Form 95 is the Federal Court of Australia application to fix the terms of a Commonwealth defence or security use of a circuit layout. It is made under s 25(4) of the Circuit Layouts Act 1989.

Form 83 is the Federal Court of Australia application for an inquiry into irregularities in an amalgamation ballot. It is made under s 69 of the Fair Work (Registered Organisations) Act 2009.

Form 84 is the Federal Court of Australia form for an inquiry into a withdrawal-from-amalgamation ballot under s 108 of the RO Act. Such inquiries now go to the Fair Work Commission.

Form 85 is the Federal Court of Australia application for an inquiry into an irregularity in an election for office in a registered organisation. It is made under s 200 of the RO Act.

Form 66 is the Federal Court of Australia originating application for judicial review under the ADJR Act. It is filed under rule 31.01 of the Federal Court Rules 2011.

Form 96 is the Federal Court of Australia application for interim relief in support of a New Zealand court proceeding. It is made under s 25 of the Trans-Tasman Proceedings Act 2010.

Form 50 is the Federal Court of Australia application for an order in the terms of an arbitration award. It is used under rule 28.14 where the Court did not refer the matter to arbitration.

Form 134 is the Federal Court of Australia originating application to register a foreign judgment under s 6(1) of the Foreign Judgments Act 1991 (rule 41.62).

Form 53 is the Federal Court of Australia application for court relief under the UNCITRAL Model Law on international arbitration. It is also used to set aside an award under article 34.

Form 69 is the Federal Court of Australia originating application for relief under s 39B of the Judiciary Act 1903. It seeks remedies such as mandamus, prohibition or an injunction against Commonwealth officers.

Form 70 is the Federal Court of Australia originating application for review of a migration decision under s 476A of the Migration Act 1958. It must be filed within 35 days.

Form 108 is the Federal Court of Australia application to review a Native Title Registrar's refusal to register a claim. It is made under s 190F(1) of the Native Title Act 1993.

Form 51 is the Federal Court of Australia application to stay a court proceeding in favour of arbitration. It is made under s 7 of the International Arbitration Act 1974.

Form 19 starts a class action (representative proceeding) in the Federal Court of Australia under Part IVA of the Federal Court of Australia Act 1976. Rule 9.32 requires it.

Form 52 is the Federal Court of Australia application to enforce a foreign arbitral award under section 8(3) of the International Arbitration Act 1974.

Form 58 is the Federal Court of Australia application to enforce an ICSID investment award under section 35(4) of the International Arbitration Act 1974.

Form 109 asks the Federal Court of Australia to order an Indigenous Land Use Agreement removed from the Register (Native Title Act 1993 s 199C(2)).

Form 107 is the Federal Court of Australia originating application for Native Title Act 1993 applications other than main applications. Rule 34.107 requires it.

Form 56 asks the Federal Court of Australia for an order when someone fails to assist an arbitral tribunal (International Arbitration Act 1974 s 23A).

Form 57 asks the Federal Court of Australia to stop or allow disclosure of confidential arbitration information. It is made under ss 23F and 23G of the International Arbitration Act 1974.

Form 100 asks the Federal Court of Australia to enforce a New Zealand court order under section 58(2) of the Trans-Tasman Proceedings Act 2010. Rule 34.71 requires it.

Form 116 starts a discrimination case in the Federal Court of Australia after the Australian Human Rights Commission terminates a complaint (AHRC Act s 46PO).

Form 81 is the Federal Court of Australia application claiming adverse action because of a protected attribute, in breach of Fair Work Act 2009 s 351(1).

Form 79 is the Federal Court of Australia originating application for a dismissal in breach of a general protection. It is under Part 3-1 of the Fair Work Act 2009 (Cth) and rule 34.03 of the Federal Court Rules 2011.

Form 80 is the Federal Court of Australia application claiming unlawful termination of employment in breach of section 772 of the Fair Work Act 2009.

Form 30 is the receiver's guarantee filed in the Federal Court of Australia under rule 14.22 when a receiver's appointment depends on a guarantee being filed.

Form 9 is the referral certificate the Federal Court of Australia issues under rule 4.12 to refer a party, often one without a lawyer, for pro bono legal help.
